All-on-4® is a surgical and oral rehabilitation technique which objective is to replace all teeth (all) on top of four (on-4) implants on the same day. There are various causes for a patient to lose teeth either on the upper maxilla or the mandible. Among these we find caries, fractured teeth and periodontal or gumdisease.By definition the All-on-4 ® technique must have the following characteristics:

It features 4 implants that support a fixed restoration, a restoration that cannot be removed from the patient mouth. Facilitating daily living.
Angled implants especially the most posterior ones to:

avoid anatomical structures.
To be able to restore the posterior teeth as far back as possible making your bite look more natural.

The fixed restoration must be installed the same day that the implants are surgically placed or within 24 hours. (immediate load).

This technique was developed by one of the largest and well known implant companies called Nobel® although some other companies have protocols and similar names such as all-on-X, full arch reconstructions, teeth in day, etc.

Avoid bone grafting procedures: The angled implants help to avoid anatomical structures such as the maxillary sinus which is located in the upper maxilla on the posterior aspect. With the All-on-4® Dr. Abril offers to his patients implants without the need of grafting. This grafting techniques may take more time in healing and can limit the the treatment to a removable denture.

Shortening the time form the implant surgery to the restauration: When a dental implant is placed in a native bone (without bone graft) the implant stability will be increased, this will make the delivery of the (temporary) restoration possible. The conventional technique used in the past was to use bone grafting techniques followed by 3-4 months of healing, then an implant was surgically placed and then another 3-4 months before the fixed restoration was installed.

All-on-4® (todos-sobre-4) es una técnica quirúrgica y de rehabilitación oral, cuyo objetivo es reemplazar todos los dientes (all) sobre cuatro implantes (on-4) en un solo día. Hay varias causas por las cuales un paciente pierde sus dientes ya sea arriba en el maxilar superior o abajo en la mandíbula. Entre estas causas están caries, dientes fracturados y enfermedad periodontal o de las encías. Por definición el tratamiento All-on-4® debe reunir las siguientes características:

Consta de 4 implantes que soportan una restauración fija es decir no existe la necesidad de retirar la prótesis de la boca, facilitando el diario vivir.•Los implantes posteriores son angulados para:

Evitar estructuras anatómicas.
Poder restaurar los dientes lo más atrás posible haciendo lucir tu mordida de una manera más natural.
La restauración fija es instalada el mismo día o dentro de 24 horas en que los implantes han sido puestos quirúrgicamente (carga inmediata).

Esta técnica fue desarrollada por una de las empresas más grandes de implantes llamada Nobel® la cual fue pionera en esta técnica a pesar de que hoy en día hay otras compañías que poseen protocolos y nombre similares como all-on-X, reconstrucción de la arcada total, dientes en un día, etc.

Beneficios de la cirugía All-on-4® :

Qué ventajas ofrece esta técnica cuando la comparamos con implantes puestos de manera convencional?

Evitan procedimientos de injertos de hueso: Los implantes angulados ayudan a evitar estructuras anatómicas como el seno maxilar que está localizado en el maxilar superior en la parte de atrás. Con la técnica de All-on-4® Dr. Abril ofrece a sus pacientes implantes donde podemos evitar injertos de hueso que toman más tiempo en cicatrizar y nos podría limitar al uso de prótesis removibles por un buen tiempo.

Reducen el tiempo desde el implante hasta la restauración: Al colocar implantes en hueso nativo (sin injertos), incrementamos la estabilidad de ese implante haciendo posible que la restauración (temporal) pueda ser puesta el mismo día de la cirugía. Esta restauración a pesar que es temporal es fija es decir permanece en su boca.

La técnica convencional usada antes era usar injerto, esperar 3-4 meses mínimo, después colocar el implante y esperar otros 3-4 meses antes de poder poner dientes fijos que no necesiten ser removidos de la boca. Con la técnica de All-on-4® el paciente podrá disfrutar de sus dientes el mismo día que sale de la oficina. Evitando así meses con dentaduras de quitar y poner.
